If anyone or their caregiver thinks services may be needed in the near future, it is best to apply as soon as possible in case there is a waiting list. The process can be daunting, but thousands have benefitted from it, and if successful is well worth the effort. Friends, family, and community members may serve as supports while navigating the system.
1-800-432-2080 is a hotline for a case manager to help identify needs, qualifications, and procedures for applying for the different programs. There may be an extended wait time for this call.
Depending on the program, applications can be submitted immediately. At some point, medical and other records may be requested.
One important benefit can be service hours by a home care aide, through the COLTS program – Centennial Care Waiver Coordination of Long-Term Services.
